Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease of unknown etiology affecting over 1% the US population. Deep epidermal infiltrative cysts and tunnels characteristic develop in intertriginous skin, with mixed immune infiltrate present inflamed regions, dominated by B T cells, but also including myeloid cells. Recent studies show progressive increase cells plasma as HS lesions evolve towards end-stage disease. Moreover, transcriptomics ex-vivo patient data demonstrates that anti-TNFα therapy induces marked decrease cell compartment. Increasing bodies support role derived autoantibodies driving aspects HS. However, limited clinical only modest significant improvements symptoms inflammation anti-CD20 treatment. Further analysis revealed non-response associated hyperactive BCL2 genetic variant heightened molecular signature. These findings suggested for Bcl-2 family function responses Bcl-XL expression lesion significantly upregulated using histology RNA-seq analysis. Notably, infiltrating T-cells express both Bcl-XL, whereas diseased keratinocytes demonstrate dramatic shift from to Bcl-XL. Taken together, these suggest inhibition Bcl-2/Bcl-XL pathway, targeting pathogenic compartment, might effectively treat patients, non-responders.
